i want to apply for my child do i put her name in the evaulatin form or my name and hoe do i apply

i need to know can kaylar lee get any assitance with her being now diagnose with a desiease cause her joints and bone to fell before she turns twenty years old of age she on medication for the rest her life has tryed before toget some assitance and was turned down cause of her learning disability can someone please help me give me a call

I have been stressing a lot I suffer from a couple of disorders like Borderline personality disorder self inflicted suicide, alcohol depended as well as anxiety disorder. May I please get the help necessary for this matter.
